Leave Of Absence Form Process

To complete your Leave of Absence Form, please follow the below steps:

1. Log in to your Prestige Student account

2. On the left-hand side of the start page, click on the link called "Reference"

3. Next, click on the link for "Forms"

4. In the navigation links menu on the right-hand side of your screen, click the link that says, "New Leave of Absence Request"

5. Complete the student section of this form as appropriate

6. After signing the form, click on "Add Instructor" and select your teacher's name, then click "Add". Next, click "Close".

7. Finally, click on the button called, "Submit form".

8. Admissions will notify you once the form has been received.
